Registration

Our registration process is simple and personalized:

• Start by contacting us via phone or email

• We'll schedule an in-person meeting to discuss:

  • Your family's needs

  • The individual receiving services

  • How we can best serve you

• This meeting is your opportunity to ask any questions and get detailed answers​

Assessment is about watching a person in different parts of their life. We do this to see what skills they have and what behaviors we want to focus on. This gives us a starting point. 

Person-centered planning is an approach to creating behavior change programs that focuses on the individual receiving services. It involves:

• Understanding the person's unique motivations and interests

• Respecting the individual's preferences and values

• Including input from people important to the individual

• Designing programs to improve quality of life based on personal goals

• Tailoring interventions to fit the person's specific needs and circumstances

This method ensures that behavior change strategies are customized to each person, rather than using a one-size-fits-all approach.

In a similar way, goals are developed based on input from the stakeholders involved along with what assessment results suggest are areas of need. Once goals are developed, approaches and strategies are created and data collected to help ensure goals are being achieved.
